Medication forms

For medications to be given at school, a Medication Authorization Form needs to be filled out by a Healthcare provider and signed by a parent/guardian.


This form can be used for daily medication, or over the couther medications as needed (Tylenol, Ibuprofen, etc.). Any prescription medications must be brought in by a parent/guardian of that student. Students are NOT allowed to bring in their own medications. For any questions, please feel free to contact Nurse Kelly.

Emergency Action Plans

Listed here are the Emergency Action Plans for Asthma and Allergies. Most local doctor's office's will also have these available. Please bring your plans in with the Healthcare Providers signature at the beginning of the school year.

 

Also listed is the student contract that is required to be signed by the student as well as the parent/guardian in order for students to be able to carry their own medication.

If your student has a chronic condition that requires an Emergency Action Plan not listed, please reach out to your healthcare provider or Nurse Kelly for the appropriate forms for school.

Sports Physical Form

Student who wish to participate in school sports will need a sports physical on file to play. These physicals will be turned in to the Athletic Director.
