SIXTH GRADE

  • The Health History Update form is a form recommended parents/guardians fill out and turn in for all rising sixth graders.

  • Sixth and Eighth graders every year have their vision screened for long distance vision. The goal of these screenings are to help detect any vision problems your child may be experiencing. If your child receives a referral letter from their vision screen, know that this is not a diagnosis. They should secure care with an eye doctor to be further tested.

  • If you are unsure where or how to secure care, please contact your school nurse so that they can help navigate you. Click here for a list of local eye care providers and the insurances they accept.

SEVENTH GRADE

  • All Students entering seventh grade must have received their updated Tdap and Meningococcal vaccinations. These are required by state law for students to attend public school.
