Welcome to Dare County Schools. We are fortunate in Dare County to have a school nurse stationed in every school and generally available to students Monday through Friday during school hours.

 

New students starting from out of state will have to have a North Carolina Health Assessment completed by a physician and provide their updated vaccination records. Below, you will find forms for the NC Health Assessment and a letter which includes all required vaccinations. If your child has a health concern you would like the nurse to know, please also fill out the Health History Update form.

 

Forms for Emergency Action Plans and Medications Administration can be found under the "Student Forms" tab. If you have any questions or concerns, please feel free to contact your school nurse.
